1.  Print and complete the Application For Mailbox Rental

2.  Print and complete the USPS Form 1583 (.pdf format);have your signature notarized.

3.  Print and complete the Customer Information Sheet.

4.  You will need to provide us with photocopies of two forms of identification. One must show your photograph and signature, while the other needs to have a home address that matches Form 1583.

5.  Payment is by credit card only, we accept Visa and Mastercard.

6.  Mail or Courier service the completed Application For Mailbox Rental, USPS Form 1583,  Customer Information Sheet and credit card information, along with copies of your I.D. to:
