Additional Terms
1. Agency discount (commission) forfeited if not paid within thirty (30) days from the date of invoice.
2. All past due invoices subject to a finance charge equal to 1.5% per month (18% A.P.R.) of outstanding balance.
3. Creditor reserves the right to employ a collection agency and/or attorneys to recover past due balances: therefore, such accounts will be subject to a reasonable attorney/collection fee equal to thirty percent (30%) of the outstanding balance.
4. All past due invoices will be subject to the "Short Rate." Any past due invoice(s) will be recalculated to the higher amount due for the frequency actually printed. "Short Rate" adjustments will also be made for any change (downward) in frequency agreement originally contracted for, either written or verbal.

Insertion Orders: Insertion orders must be received by the Space-Close Deadline. If a change of copy is not received by the Materials Deadline, the contracted advertiser's copy that ran in the previous issue will be inserted.
Cancellations: Cancellations will not be accepted after the Space-Close Deadline. Cancellations for the cover ads must be made 90 days prior to the scheduled Space-Close Deadline.

Materials: Materials must be received by the Materials Deadline. Advertising materials will be returned at the advertiser's request. The publisher is not responsible for advertising material left more than three months.

Digital-Format Submissions: Before submitting ads in a digital format, please request our Digital Requirements to make sure your submission is compatible with our system and assure quality reproduction. If not, your advertisement may be rejected. An appropriate press proof must be submitted with all digital ads.
Film requirements: Negative film, right reading emulsion side down, minimum 133 line screen, color 150 line screen preferred. All film must meet SWOP standards. Supplied negatives must be accompanied by appropriate proofs.

Contract/Copy Regulations
Publisher reserved the right to hold advertiser and/or its advertising agency jointly and severally liable for such monies as are due and payable to the publisher.
Publisher reserves the right to change rates at any time.
Publisher accepts no liability for damages due to advertisements not appearing in contracted issue.
All advertisements are subject to acceptance by the Publisher.
Publisher also reserves the right to decline or reject any advertising for any reason, at any time without liability, even though previously acknowledged or accepted.
Publisher shall not be liable for copy and/or key changes unless provided with complete replacement negative.
Advertising Responsibility: The purchaser of the advertising space assumes all responsibility fro the entire content of any such advertisements printed in Forza and assumes responsibility for any claims or costs if litigating claims arising therefrom against the Publisher.
Publisher will not be bound by conditions printed or appearing on order blanks or copy instructions which conflict witht he provisions of the stated rates.
