Please allow this student to take one of the 3 HTML Certification Exams which I use in CIS 58.54.

I need you to do the following things

1. Check the student for ID and then list their name here

name of student_____________________________

proof of ID ________________________________

name of lab staff __________________________

2. Make sure that when they take the exam they are not using notes or books.

3. Please go to the CATE website

Then allow the student to take the exam and print out the page which shows their score.

Please attach this form to the print out of your score which will come up after you take the test. Put this in my box in the Santa Rosa lab or office boxes.
